Late, late, for a very important pill?

Forgot to take your pill on time? Stay calm and read on...

by Andrea Jackson, MD

The pill is a great form of birth control, provided you remember to take it. You may have seen information about “perfect use” stating that the pill is 99.7% effective. (That means that of 333 women taking the pill for a year, one will become pregnant.) “Perfect use” assumes a woman takes the pill at the same time every single day and never misses one.

But who’s perfect?

In the real world, it’s a challenge for anyone with a daily medication to take it perfectly. Even people with serious medical conditions like high blood pressure have a hard time taking medicine every day. The World Health Organization estimates that about half of people with chronic medical conditions take their medication late or skip it completely on some days (PDF). In a study of young women using the pill, the majority (80%) missed one or more pill within the first few months.

That’s why the “typical use” effectiveness for the pill is lower: 91%. (That means that of 11 women taking the pill for a year, one will become pregnant.) If you take the pill, your doctor probably recommended that you take it at the same time every day. Making it part of a daily routine can help you remember. Some women keep the pill packet next to their toothbrush, or get fun text message reminders every day.

What should I do about a late pill?

When your routine is a little off, you can still protect yourself from an accidental pregnancy. Exactly what to do depends on three things:

1) What type of pill you use. If you’re on the mini-pill, a.k.a. the progestin-only pill, ignore the chart and see the section on mini-pills below.

2) If you’re a combination pill user, as most pill users are, it depends on whether it’s the first week of a new pill pack, and

3) how long it’s been since you took the last pill.

I’m using the mini-pill—is that different?

About 1% of women taking the pill in the US are using the mini-pill, or progestin-only pill. The active ingredient in mini-pills doesn’t stay in your body as long as combined pills, and there are no placebo pills. If you are late or skip a mini-pill at any time, follow the instructions above and use a back-up method like a condom (or EC if you miss the condom) if you have sex in the next two days. Yup, with mini-pills you have to use back-up even if you’re only a few hours late.

Um, you didn’t answer my question

If it’s not urgent, ask below and I’ll get back to you! If it is urgent, call your doctor or a local clinic for help. If you can’t get through on the phone and you need help right away, you can also ask a pharmacist at a drug store.

If I took my pill 3 hours later than I usually do and had sex that night will it still effective?

I missed 3 pills this week. Monday, Tuesday, and Thursday. It is about 10pm on Friday night, and I got my period this morning. I just had my period, though. It will be two weeks on Sunday that that I started taking the blue pills again. (I'm on week 2 of my BC right now) Should I take my pill for tonight, even though I missed 3 days and am having my period right now? Or should I not take any pills for the rest of this month, and start over again on the 1st of next month? Or, should I not take any pills till my period is over, and then start again? My boyfriend is out of town, and will be for the next 6 months, (which is why I missed those three days, what with all the crying and feeling sorry for myself I've been doing), so it is not particularily urgent that I be on BC, but I tend to have really wonky and stupid periods, so I would like to be normal again! Please help!

I have taken this pill once everyday for the last 3 months and I am two weeks late on my period. Why is that?

It's difficult to know based on what you wrote—it sounds like you had a month in which you missed two pills, then had some bleeding, then had sex without a condom at some point after the bleeding started. Your risk for pregnancy depends on whether the bleeding was your normal period or bleeding at some other time. You can always take emergency contraception within 5 days (the sooner the better, though) to reduce the risk of pregnancy after unprotected sex. I'd also suggest setting up a reminder (http://bedsider.org/reminders) to help you remember your pill each day.

What happens if you miss one pill in week two and never make it up and took the next at 12 in the morning and it was during your "fertile" and "ovulation" week???

I have been on birth control for about a year now and I have missed a pill but have always taken it with the following pill the next day. I had irregular periods/spotting the first 3 months on the pill but my body got use to it and began having regular monthly periods. About 3 months ago I stopped getting my period. I took a pregnancy test at home that came out negative. Is the only reason for my missed periods pregnancy or is there another reason this could be happening?

im on combination pilss and took 2 pills, 4-5 hours late, 2 days in a row. They were the end of second week/beginning of 3rd week of active pills. I also had unprotected sex 4 days later. should i be worried?

Hi Jenn, thanks for your note. Up to one in 20 women using the pill for more than a year have no period. However, it's possible that there is another reason that you stopped getting your period (e.g. changes in your body's hormones), so it's a good idea to check in with your health care provider. If you find that your pill is the culprit and you dislike not having a period, your provider can also help you switch to a different pill or another method of birth control.

I hadn't taken my pill for the entire day and then took it only an hour before having sex with no other protection... what are my chances of pregnancy?

"My last pill was less than 24 hours ago" is a bit confusing to me. If I took one less than 24 hours ago, my next one isn't even due yet? But then it goes on to say it could be 2 or 20 hours late. So does this actually mean as long as the pill isn't 24 hours late? I am aware that the combination bcp's can actually be taken every 36 hours but are taken daily for simplicity--so you'll get used to taking at the same time every day. So if you forget one for 12 hours or less, you're most likely fine. I forgot one for 14 hours yesterday. Meaning that the last one I took was the night before last or 38 hours ago. I'm in week three of the active pill. I took it as soon as I remembered and I think I'm most likely fine but I'd like to know in terms of percentage what my risk is now. Is the pill only 91% effective now? If sperm can live inside my body for 5-7 days and this allows ovulation, I could get pregnant even if I use a back up method for 7 days? How soon would I ovulate if taking one late does cause ovulation?

I normally take mine at 11 pm, and I've only been on it since last Thursday. I fell asleep early last night and didnt take it but I took it this morning at 7:40 am. I'm like 95% sure I'm having sex with someone on Saturday. Do I need to use backup protection?

I use the three month pill pack. While on vacation I realized the pack had been left behind at the last hotel. When I realized it was night time and earlier that morning I had unprotected sex. I had no way of getting the pills until I returned back home. I went three days without the pill but didn't have sex except for the incident when I didn't know. I only had three pills left in the pack. How likely is it that I could be pregnant?

I missed three pills (Thurs/Fri/Sat) in the first week and doubled up on Sunday only. I began bleeding on Sunday (after just finishing a period 10 days earlier). I had sex before, during, and after the missed pill days without protection. I am now on my fourth day of spotting. Is it possible to ovulate and get pregnant while bleeding?

What're the chances of becoming pregnant if you take the pill (I've taken the pill regularly and never missed one day for the past year and half), have sex without a condom (in which he does sperm inside of me), and then days later stop taking the pill?

I didn't miss my pill for the first month, and I started the second month. I took my pill and had unprotected sex (in which he released semen inside of me) at the end of the first week, and completely forgot to take my pill the next night (beginning week 2). DId my odds of getting pregnant become greater than the typical "5% more likely when you miss the pill"?

I took microgestin for 3 weeks. I had unprotected sex throughout the entire month (via the pull out method) except 2 days before I quit taking them and the day I quit. those 3 days he did not pull out at all. I did not take the very last active pill or the sugar pills to follow. My question is, could I have ovulated a couple days after I stopped taking the pill and got pregnant? I am, according to my last period, 9 days late. So if I did in fact ovulate right when I came off the pill, it would've just implanted a few days ago I think. My cervix was high last week and is even higher now. going to see my doctor Monday if I have not started by then. what do you think?

This If Going To Sound Stupid, But, I Was Taking My Birth Control At The Same Time In The Morning Everyday. I Did, However, Miss It... A Few Times. I Found Myself Doubling Up More Than Once This Last Packet. I'm Not Due For My Period Until This Upcoming Monday But I Had A Concern About Something: Say I Were To Have, Inadvertently, Conceived. But Because I Didn't Know It, I Continued Taking The Pill And Doubling Up More Than Once- What Kind Of Damage Could That Cause The Fetus? Or, Could I Cause An Abortion? I Appreciate Any Advice You Can Give Me. Thank You.

2014-01-04 07:47:09 UTC

Claire

Hi Natasha - Yes, any time a woman misses two or more pills and has sex, there's a risk of pregnancy. The more missed pills, the higher the chance of pregnancy. I can see how another 4-hour trip to pick up the pills wouldn't be so easy! If your pill is affordable, consider getting more than one pack at a time and stashing one at home for cases like this.

2014-01-08 01:38:44 UTC

Andrea Jackson

Hi Sara - It's difficult to say what the chance of pregnancy is based on what you wrote. It would mostly depend on how many days passed between having sex and stopping the pill, and it might also depend on where in the pill pack you stopped taking them. The more days between sex and stopping the pill, the lower the chance of pregnancy.

2014-01-08 01:40:55 UTC

Andrea Jackson

Hi mgirl, Yes, it is possible that missing three pills can lead to pregnancy, regardless of whether a woman is experiencing a withdrawal bleed.

2014-01-08 01:41:59 UTC

Andrea Jackson

Hi ellafromupnorth, Unfortunately, we don't have the studies we'd need to answer your question with certainty. We can only say that missing three pills increases the risk of pregnancy.

2014-01-08 01:45:04 UTC

Andrea Jackson

Hi Katie, Based on what you wrote, it seems unlikely that you are pregnant. Your bleeding may have started early because of the missed pills. Back up your birth control pill with another method for the first week of your new pill pack.

Hello everyone, I just wanted to ask a couple of quick questions. I have been taking my birth control pills every day for 3 weeks, not at the same time. I took my Friday pill at 3pm, had unprotected sex that night. I missed my Saturday pill completely because I didn't get home until Sunday night. I took TWO pills that Sunday, one at 7pm and one at 10pm.... Is there a chance I could be pregnant? Can I still take the day after pill today, Monday?

Hello ladies I want to know if there's a chance I may be pregnant. I took my pill regularly (not at the same time every day) until a Friday night. I had unprotected sex that Friday night and my partner finished inside of me. Saturday I completely forgot to take my pill so I took 2 pills Sunday, one at 7pm and one at 10pm and continued to take them regularly after that for a week now. I want to know if there's a chance I may be pregnant so that I stop taking my pills since there is no need and I'm afraid they'll hurt the fetus.

I typically take my pill around 9 pm and totally forgot last night(Tuesday), and took one this morning at 7 am. Now it was dark in my room and I was half-asleep so I accidentally started week 3 by taking the week 3 pill for Tuesday instead of Week 2. I called my pharmacist about it and she told me to continue with Week 3. Now does this mean that when I have sex I should most definitely use a condom? Or is it still effective enough to prevent pregnancy?

2014-01-22 14:34:19 UTC

CanaryTB1

Hi Canary! Good job realizing your mistake and calling your pharmacist, because the answer to your question can depend on the specific pill you're using. In general, when a woman has trouble with a pill, it's a good idea to add a second method, like a condom, for double protection. It's also a good time to think about switching to a longer acting method so you don't have to worry about this again!

Hello, I had missed my first pill of the first week and had unprotected sex the night after (it occurred around 5pm. I normally take my pill around 7pm) and I took the first two pills (the one I missed and the one I was originally supposed to take that night) an hour after sex (around 6pm). Am I at risk of pregnancy?

Hi I just started the birth control pill this month and already started my next pill pack but I accidentally took my placebo pill with an active white pill on the same day but at different times, is there a risk of me getting pregnant the next time I have unprotected sex? Im just curious since it is my first time.

I take the mini-pill. I haven't had sex since Sunday night. On Tuesday night I forgot to take it. Just took it Wednesday afternoon at 4pm. Is it likely that I will be pregnant ?

I have been on birth control for 2 years now, and the other day I was about to take my BC pill and it literally went down the drain. (the other day was last Tuesday) The next day I got right back on schedule. So, of course I told my boyfriend we needed to use protection for a little while. Well, last night we had sex and he accidentally came inside me. Yesterday was the 8th day since I missed my pill, but I am still nervous that I might be pregnant. Please help!

I set up to take my pills at 1030 pm everything but this month on the second day of the first week I fell asleep early and didn't take my pill I woke the next morning and I took it. Which was about 9 hours later about. Later on through the month I slept at a family members house and forgot my pills at home this was the Saturday of the second week and Sunday of the third week...the Wednesday of the third week I got my period but it's extremely light associated with cramping, food cravings ( I'm constantly looking for something to eat) headaches, nausea, bloating, fatigue and earlier on my breasts were tender. Is there a possibility of me being pregnant. By the way I've been taking birth control for a few months now this was the first time I've been late or forgotten

2014-02-21 05:29:24 UTC

Jamie

Hi Countrygirl, Assuming you are on a combined hormonal contraceptive pill, you are protected from pregnancy. That's because you haven't missed any pills, just taken them a few hours late. A "missed pill" is taken 24 hours or more after the scheduled time.

2014-02-21 20:35:00 UTC

Andrea Jackson

Hi Lee--sorry for the slow response. For future reference, if your girlfriend took her pill late but has not skipped any pills, she should be covered. That said, you should only do what she's totally comfortable with in terms of protected or unprotected sex. Remember the pill does not protect against sexually transmitted infections.

2014-02-21 20:36:08 UTC

Andrea Jackson

Hi Anna, Sorry, we just don't know enough to give you a precise estimate of the increase of your risk of pregnancy. In general, the more missed pills, the more likely an accidental pregnancy.

2014-02-21 20:36:40 UTC

Andrea Jackson

Hi amber--sorry for the slow response. So long as you haven't missed any pills, you are protected from pregnancy. It's normal for the pill to cause lighter, shorter periods.

2014-02-21 20:37:06 UTC

Andrea Jackson

Hi joe--sorry for the slow response. Missing 4 pills in a row means you are not protected against pregnancy, so it's possible that you could get pregnant. In the future, you have 7 days after unprotected sex to get a copper IUD as emergency contraception--or ella (a type of morning-after pill) is effective up to 5 days after unprotected sex.

I've been wondering about this for awhile. I've been on the pill for over two years. It is on a 28 day cycle (21 active pills+7 placebos), but my body is naturally on a 25 day cycle. So I'm much more regular *without* the pill than with it. A lot of times my bleeding overlaps when I'm on my active pills. My question is: can I alter the pack to be closer to my natural cycle? Like take 19 active pills+6 placebos (25 total)? Would this still keep me protected? I've heard of other people doing it, just wanted to be sure. Thank you!

Hi Angel. I'm not sure what you mean by 'delayed', so I'll answer generally, okay? You can always call the provider who wrote your prescription with questions, too! A woman can start the pill anytime in her cycle. Depending on where you are in your cycle, you may need to use a condom to be fully protected for a week or two after starting.

Hi Cheyenne - This is a hard situation, since you've been sick, and missed a whole week of pills - two things that can contribute to an unplanned pregnancy. It's important to know that you are DEFINITELY at risk right now for getting pregnant, and so you should use a backup method until this is sorted out. There's not good data on what to do in this situation, but most providers would probably tell you to just start taking one pill per day, use a backup method for about seven days, and know that the next cycle may have some unscheduled spotting. Also, this might be a good time to consider a method that won't be affected by a bad stomach bug, like an IUD or subdermal implant. When you're sick, you have enough to worry about!

I've taken my pill every day at the same time for three years. I take a normal pill, not a mini pill. I skip my placebo pills every month so I only take the pill daily. Yesterday I missed my pill and there was a 26 hour gap before I took it. I took my normal daily pill 3 hours later and had sex 20 min after, could I get pregnant?

Hi there! I just want to know if i'm at risk of getting pregnant. I'm currently using mini-pill (excluton) fo almost 2 months now. So here's the scenario: Friday 12mn - Took a pill Saturday 12mn - Took a pill Sunday 12mn -Took a pill Sunday 4pm - Unprotected sexual Intercourse Monday 12mn - took a pill Tuesday 12mn - Missed to take the pill Tuesday 6am - took a pill Am I still protected or should I be worried? Note that I never miss to take the pill at the same time before Tuesday. Also on Sunday at 6pm, I had a menstruation-like discharge but stopped the same day (I had it also around same time last month) Thank you in advance!

I forgot to take my birth control (TriSprintec) at my regular time 10pm and did so 2 1/2 hours late at 12:30am. The pill was the second in the pack on the first week. Earlier that day my boyfriend and I had unprotected sex. My period app is telling me I am fertile. Was the pill still effective, should I get the morning after pill?

I've only been taking my birth control or "mini pills" for 38 days. Within the first week of my second pack of 28 pills I had sex. Am I at the risk for being pregnant? We had sex twice and both times he didn't use a condom. I usually am bad about taking my pills and end up taking them an hour or two late.

You can read more about the method here, but the bottom line is that it has about a 22% failure rate. Whether this is a good method for you and your partner depends on how important pregnancy avoidance is to you two right now. http://bedsider.org/methods/withdrawal#details_tab

I take my pill, Generess Fe, at 10:00 P.M. I have been on time during this whole pack and last pack. Last night, I fell asleep before I took it. I woke up this morning around 8 and took the pill. About an hour later, I got sick and threw it up. My boy friend was taking care of me.. And we ended up having sex. He did 'go' in me as well. when I got home, I immediately took another, which was at 1:00 P.M. 15 hours late. Should I go ahead and go get the Plan B? Or should I be okay?

Hi, I had taken my third month of microgestin and During the week you take off I got my period for three days and then had sex (but he didn't Finnish because I felt pain) I went to the ob and he said I had a ruptured ovarian cycst. This was causing the pain...so me and my boyfriend stoped having sex for two weeks to give my body a chance to heal. During this two weeks I stoped taking the birth control. We had sex on a monday, and then the next Sunday I started my new pill pack. Now that means that technically I should have gotten my period a week after I started the new pill pack, but because of the pill pack (at least I hope) I'm like 14 days late for my period...at this point I'm not sure if I could be pregnant...we've had sex a few times since we started back up and it was un protected but he never cums inside me. Were both really worried, and any insight would be helpful...I'm not sure why I'm so late on my period...I think it might just be because of the pill pack being started late, but I'm not sure...

On Monday night I had unprotected sex with my boyfriend and he pulled out. Please help! I take Orsythia and I was late taking it Monday by about 3 hours. I was late again Tuesday and took it about 4 hours late. On Wednesday I took I about 6 hours late. Today (Thursday) I took it at the normal time and had unprotected sex today. Is pregnancy likely under these circumstances??

I take tri sprintec. I take my pill at 11pm. In the 3rd week I missed a pill I didn't realize until I went to take the next pill the next day which that pill was already like an hour late. The next day after I had taken 2 pills I was about 13 hours late and I had unprotected sex. What are the chances I could be pregnant. Should I be scarred. I've been I'm the pill for about a year and a half.

Hi Sarah, and hope all is well, since this reply is late! Following the directions above can be helpful and its important to double up on protection with condoms if you're not sure. It can be hard to remember pills, so remember there's plenty of non-daily methods, like the IUD or implant.

I was at the end of my second week when I had unprotected sex multiple times (over the course of 2 days) with my fiancé & took my BC pill on those days, but for the 3 days after we had sex, I forgot to take the pill. Is there a great risk or small risk of pregnancy? After those 3 days, I got back on the schedule and have been taking them for the past 4 days regularly.

Okay because I had the same problem . I forgot my pills at home when I went to go shop and have dinner...so I took it late (I usually take it at 4/4:30 everyday!) But yesterday I took it super late like at 8...then I had sex this morning and it was protected, but the condom came of inside of me, so a little got in me...I urinated right after but I wasn't sure if I could get pregnant or not, and yesterday I took my second to last pill of my pack and today, I have to take my last pill of the today so, that I can have my period naturally for seven days.. So am I okay?...

I missed my pill Monday and just didn't take it at all. I had "sex" today (well he went in twice-ish but I stopped him after that) I' haven't missed since and yeah should I be okay?

I take my pills at 7:05 in the morning everyday. This past Tuesday I forgot to take my pill and ended up taking it in the night. Today I also forgot my pill and ended up taking it an hour late. I had unprotected sex the day before. For this month, I miss the two pills from my third week. What should I do?

I have been on ortho try cyclin for a few months. I usually take my pill at 6am. After the last 7 days of inactive pills, I took the first pill at 5pm and the next day, I took the second pill at 5pm. Then I resumed my morning 6am time. On day 20 I had unprotected sex with my bf and he finished in me. Was I still protected even though I screwed up those first 2 days? Also, how rigid is the "same time every day"? Would I still be protected if I took it at 6am one day and 7am the next?

Nicole - Pills are advertised as over 99% efficacy, but in real life, almost 1/10 women become pregnant while using the pill. Factors like irregularities in dosing certainly can contribute to decreased efficacy in real life(along with many other factors). It's hard to give a scientific answer to how important it is to strictly follow dosing of pills, but if you're relying on the pill as your primary contraceptive, I can't in good conscience recommend anything but adhering to the recommended dosing. Many women find this to be unrealistic or stressful (or both) which is why there are so many methods that don't have such strict requirements.

Hi! So I took my pill about 9 hours late and then later that day started my period after I had had it only a week before. I am now on day four of the period and it has been pretty heavy the entire time even though I continue to take the estrogen pills at the regular time each day. Is this normal?

I've been on my pill for a few months&I've recently stopped taking the pill after the the first two weeks of my 21 day pack because I didn't want to get my period at the end of the month, after that week of not taking the pill if I return to regular schedule to finish the pack would there be any higher risks at pregnancy?

I'm trying to get pregnant and the only time I have ever become pregnant is when I was on birth control but not taking it like I was suppose to. I had my doctor do a bunch of lab work and everything came back fine but my esterdiol was low. Do u think if I start my birth control and skip it every so often I could increase my chances of getting pregnant

I was not able to get my pill pack on time, and I missed the first week of taking it. I missed an entire week of no pills... Is there a chance I could get pregnant while having sex that one week or later in the month? I took the pills like normal once I got my new pack.

Hello. I normally take my BC at 11:30 every night. Last night, after having sex without a back up method, I fell asleep. I woke up this morning and took my pill at 7:30 AM. (Around 8 hours late.) I take Low Ogestrel. I have never been late for a pill before. Should I take an emergency contraceptive? Please resond.

What you're describing is very common - becoming pregnant soon after stoping a birth control pill. You should talk with your doctor to see if this is a good idea for you.

Ok so here's my thing. I take my pills 11pm everyday. A thursday night i didnt take it and realized it 12 hours later and took that pill right away. Friday and saturday i took my pills like normal at 11pm. I had intercourse after 11pm on saturday night. Then i missed my sunday pill 24 hours so, i took it on monday with my monday pill 11pm. I know that sperm stays in the body for a couple days. These pills I forgot to take were week 3 in my pack. This is so careless of me because i've never ever missed an active pill before and I've been taking birth control for about a year and a half. I've missed placebo pills because i know they're placebos and it's no big deal. I'm very nervous. Please help. My husband and I are not ready for a kid. I need a response asap.

Hi so my question is kinda complicated, so I've taken a couple of pills 2-3 hours late but in different weeks and had unprotected sex in those days as well I'm on the 28 day pack so I'm not sure if I could be pregnant or not and then the last active pill of the pack was forgotten to be taken for 24 hours and then I had sex again 3 days later on the non active pills because my cycle hasn't started... Am I at risk????

Hi, i had sex 3 weeks ago and been taking the pill since, however I've been accidentally taking it every 24 hours, instead of 12. I forgot to take it 6 hours I go, so I took it and I'm waiting till my normal hour, should I decrese my hour to the regular 12 or keep taking them like I've been?.. I haven't had sex since I started taking the pills and I'm using mini-pills.
