Late, late, for a very important pill?

Forgot to take your pill on time? Stay calm and read on...

The pill is a great form of birth control, provided you remember to take it. You may have seen information about “perfect use” stating that the pill is 99.7% effective. (That means that of 333 women taking the pill for a year, one will become pregnant.) “Perfect use” assumes a woman takes the pill at the same time every single day and never misses one.

But who’s perfect?

In the real world, it’s a challenge for anyone with a daily medication to take it perfectly. Even people with serious medical conditions like high blood pressure have a hard time taking medicine every day. The World Health Organization estimates that about half of people with chronic medical conditions take their medication late or skip it completely on some days (PDF). In a study of young women using the pill, the majority (80%) missed one or more pill within the first few months.

That’s why the “typical use” effectiveness for the pill is lower: 91%. (That means that of 11 women taking the pill for a year, one will become pregnant.) If you take the pill, your doctor probably recommended that you take it at the same time every day. Making it part of a daily routine can help you remember. Some women keep the pill packet next to their toothbrush, or get fun text message reminders every day.

What should I do about a late pill?

When your routine is a little off, you can still protect yourself from an accidental pregnancy. Exactly what to do depends on three things:

1) What type of pill you use. If you’re on the mini-pill, a.k.a. the progestin-only pill, ignore the chart and see the section on mini-pills below.

2) If you’re a combination pill user, as most pill users are, it depends on whether it’s the first week of a new pill pack, and

3) how long it’s been since you took the last pill.

I’m using the mini-pill—is that different?

About 1% of women taking the pill in the US are using the mini-pill, or progestin-only pill. The active ingredient in mini-pills doesn’t stay in your body as long as combined pills, and there are no placebo pills. If you are late or skip a mini-pill at any time, follow the instructions above and use a back-up method like a condom (or EC if you miss the condom) if you have sex in the next two days. Yup, with mini-pills you have to use back-up even if you’re only a few hours late.

My boyfriend is out of town, and will be for the next 6 months, (which is why I missed those three days, what with all the crying and feeling sorry for myself I've been doing), so it is not particularily urgent that I be on BC, but I tend to have really wonky and stupid periods, so I would like to be normal again! It's difficult to know based on what you wrote—it sounds like you had a month in which you missed two pills, then had some bleeding, then had sex without a condom at some point after the bleeding started. Your risk for pregnancy depends on whether the bleeding was your normal period or bleeding at some other time. You can always take emergency contraception within 5 days (the sooner the better, though) to reduce the risk of pregnancy after unprotected sex. I'd also suggest setting up a reminder (http://bedsider.org/reminders) to help you remember your pill each day.

I have been on birth control for about a year now and I have missed a pill but have always taken it with the following pill the next day. I had irregular periods/spotting the first 3 months on the pill but my body got use to it and began having regular monthly periods. About 3 months ago I stopped getting my period. I took a pregnancy test at home that came out negative. Is the only reason for my missed periods pregnancy or is there another reason this could be happening?

However, it's possible that there is another reason that you stopped getting your period (e.g. changes in your body's hormones), so it's a good idea to check in with your health care provider. If you find that your pill is the culprit and you dislike not having a period, your provider can also help you switch to a different pill or another method of birth control.

However, it's possible that there is another reason that you stopped getting your period (e.g. changes in your body's hormones), so it's a good idea to check in with your health care provider. "My last pill was less than 24 hours ago" is a bit confusing to me. If I took one less than 24 hours ago, my next one isn't even due yet? But then it goes on to say it could be 2 or 20 hours late. So does this actually mean as long as the pill isn't 24 hours late? I am aware that the combination bcp's can actually be taken every 36 hours but are taken daily for simplicity--so you'll get used to taking at the same time every day. So if you forget one for 12 hours or less, you're most likely fine. I forgot one for 14 hours yesterday. Meaning that the last one I took was the night before last or 38 hours ago. I'm in week three of the active pill. I took it as soon as I remembered and I think I'm most likely fine but I'd like to know in terms of percentage what my risk is now. Is the pill only 91% effective now? If sperm can live inside my body for 5-7 days and this allows ovulation, I could get pregnant even if I use a back up method for 7 days? How soon would I ovulate if taking one late does cause ovulation?

I took microgestin for 3 weeks. I had unprotected sex throughout the entire month (via the pull out method) except 2 days before I quit taking them and the day I quit. those 3 days he did not pull out at all. I did not take the very last active pill or the sugar pills to follow. My question is, could I have ovulated a couple days after I stopped taking the pill and got pregnant? Hi Natasha - Yes, any time a woman misses two or more pills and has sex, there's a risk of pregnancy. The more missed pills, the higher the chance of pregnancy. I can see how another 4-hour trip to pick up the pills wouldn't be so easy! If your pill is affordable, consider getting more than one pack at a time and stashing one at home for cases like this.

Hi Sara - It's difficult to say what the chance of pregnancy is based on what you wrote. It would mostly depend on how many days passed between having sex and stopping the pill, and it might also depend on where in the pill pack you stopped taking them. The more days between sex and stopping the pill, the lower the chance of pregnancy.

Hi Katie, Based on what you wrote, it seems unlikely that you are pregnant. Your bleeding may have started early because of the missed pills. Back up your birth control pill with another method for the first week of your new pill pack.

I typically take my pill around 9 pm and totally forgot last night(Tuesday), and took one this morning at 7 am. Now it was dark in my room and I was half-asleep so I accidentally started week 3 by taking the week 3 pill for Tuesday instead of Week 2. I called my pharmacist about it and she told me to continue with Week 3. Now does this mean that when I have sex I should most definitely use a condom? Hi Canary! Good job realizing your mistake and calling your pharmacist, because the answer to your question can depend on the specific pill you're using. In general, when a woman has trouble with a pill, it's a good idea to add a second method, like a condom, for double protection. It's also a good time to think about switching to a longer acting method so you don't have to worry about this again!

Hello, I had missed my first pill of the first week and had unprotected sex the night after (it occurred around 5pm. I normally take my pill around 7pm) and I took the first two pills (the one I missed and the one I was originally supposed to take that night) an hour after sex (around 6pm). Am I at risk of pregnancy?

I have been on birth control for 2 years now, and the other day I was about to take my BC pill and it literally went down the drain. (the other day was last Tuesday) The next day I got right back on schedule. So, of course I told my boyfriend we needed to use protection for a little while. I set up to take my pills at 1030 pm everything but this month on the second day of the first week I fell asleep early and didn't take my pill I woke the next morning and I took it. Which was about 9 hours later about. Later on through the month I slept at a family members house and forgot my pills at home this was the Saturday of the second week and Sunday of the third week...the Wednesday of the third week I got my period but it's extremely light associated with cramping, food cravings ( I'm constantly looking for something to eat) headaches, nausea, bloating, fatigue and earlier on my breasts were tender. Is there a possibility of me being pregnant. By the way I've been taking birth control for a few months now this was the first time I've been late or forgotten

Hi Countrygirl, Assuming you are on a combined hormonal contraceptive pill, you are protected from pregnancy. That's because you haven't missed any pills, just taken them a few hours late. A "missed pill" is taken 24 hours or more after the scheduled time.

Hi Lee--sorry for the slow response. For future reference, if your girlfriend took her pill late but has not skipped any pills, she should be covered. That said, you should only do what she's totally comfortable with in terms of protected or unprotected sex. Remember the pill does not protect against sexually transmitted infections.

Hi joe--sorry for the slow response. Missing 4 pills in a row means you are not protected against pregnancy, so it's possible that you could get pregnant. In the future, you have 7 days after unprotected sex to get a copper IUD as emergency contraception--or ella (a type of morning-after pill) is effective up to 5 days after unprotected sex.

I've been wondering about this for awhile. I've been on the pill for over two years. It is on a 28 day cycle (21 active pills+7 placebos), but my body is naturally on a 25 day cycle. So I'm much more regular *without* the pill than with it. A lot of times my bleeding overlaps when I'm on my active pills. My question is: can I alter the pack to be closer to my natural cycle? Like take 19 active pills+6 placebos (25 total)? Would this still keep me protected? I've heard of other people doing it, just wanted to be sure. Thank you!

Hi Angel. I'm not sure what you mean by 'delayed', so I'll answer generally, okay? You can always call the provider who wrote your prescription with questions, too! A woman can start the pill anytime in her cycle. Depending on where you are in your cycle, you may need to use a condom to be fully protected for a week or two after starting.

Hi Cheyenne - This is a hard situation, since you've been sick, and missed a whole week of pills - two things that can contribute to an unplanned pregnancy. It's important to know that you are DEFINITELY at risk right now for getting pregnant, and so you should use a backup method until this is sorted out. There's not good data on what to do in this situation, but most providers would probably tell you to just start taking one pill per day, use a backup method for about seven days, and know that the next cycle may have some unscheduled spotting. Also, this might be a good time to consider a method that won't be affected by a bad stomach bug, like an IUD or subdermal implant. When you're sick, you have enough to worry about!
